RESUMO

Objective@#To understand the current status of follow up of people living with HIV/AIDS by health service at grass root in rural area of Jiangxi province and related factors, and provide references for the promotion of the follow up by grass root health service.@*Methods@#People living HIV/AIDS aged ≥18 years and diagnosed before 31 December 2017 in 6 townships of Xinjian, Yushan counties and Guixi city were included in the study in Jiangxi province. They had been followed up for more than one time after the first epidemiologic survey. The information about their demographic characteristics and HIV infection status were collected by using self-designed questionnaire. Univariate and multivariate logistic regression analyses were conducted to identify the factors that influencing the acceptance of follow up by grass root health service.@*Results@#Of the 373 surveyed HIV infected subjects aged (53.06±16.15) years, 261 were males (70.0%, 261/373). Among the surveyed subjects, the illiteracy and people who received only primary school education accounted for 54.7% (204/373). The rate of follow up of the HIV infected subjects by grass root health service was 55.8% (208/373), and those through heterosexual contact were 58.5% (190/325). The multivariate regression analysis showed that the acceptance of follow up by grass root health service was higher in those who were farmers (OR=7.36, 95%CI: 2.52-21.45), had family support (OR=16.01, 95%CI: 2.25-49.73), didn’t worry about discrimination (OR=12.97, 95%CI: 4.75-35.42), trusted health care provider (OR=5.07, 95%CI: 2.19-11.76) and showed AIDS symptoms (OR=10.58, 95%CI: 2.25- 49.73).@*Conclusions@#The performance of follow up of people living with HIV/AIDS by grass root health service was well, suggesting it is a feasible management model. Being famer or not, family member supporting or not, worry about discrimination or not, trusting health care provider or not and showing AIDS symptoms or not were the main factors influencing the acceptance of follow up by grass root health service.

RESUMO

Objective To evaluate the efficacy and safety of donepezil combined with memantine in the treatment of Alzheimer′s disease and provide a guidance for the proper clinical use of those medications.Methods The literatures published from 2014 to 2016 in CNKI/VIP/WanFang Med-online/CBM and other databases were collected.Screening and quality evalua-tion were carried out with include and exclude standard.The software RevMan 5.3 was used for data analysis.Results Eight-een studies included are randomized controlled trials with 900 cases in study group and 896 cases in control group.The com-bined results showed that MMSE,NPI and BEHAVE-AD of the combination therapy were superior to those of monotherapy group.There is no significant difference in ADL and ADAS-Cog.Conclusion The combination therapy was better than mono-therapy both in mental state and pathological behavior.The clinical application of those medications should be based on the spe-cific conditions of individual patient.

Objective A BP neural network model for diagnosing type 2 diabetic nephropathy based on laboratory tests was developed and evaluated. Methods Patients with type 2 diabetic nephropathy from 5 hospitals of Chongqing,Guizhou and Sichuan Provinces from January 2016 to December 2016 were collected in the study. Totally 89 parameters were analyzed by univariate analysis to identify significant variables by SPSS 19. 0 and MATLAB 2014a. The diagnostic performance of the two methods were compared. Results A total of 477 patients with type 2 diabetic nephropathy and 449 patients of control group were included. Univariate analysis showed that 42 variables had significant difference. Logistic regression analysis showed that 12 variables were included in the optimal regression equation. This BP neural network had 42 input layer nodes,15 hidden layer nodes and 1 output layer nodes. The Youden index of logistic regression analysis and BP neural network(training set and test set) were 0.76,0.89 and 0.83. The accurately diagnosed were 88.12%,94.24%,and 91.34%,the AUC were 0.95,0.98,and 0.96. Conclusion A BP neural network model was developed,which has important accessory diagnostic value for diagnosis of type 2 diabetic nephropathy. But all these conclusions need further validation in clinic.

Objective To analyze the risk factors of congenital heart disease(CHD) in China by using the meta‐analysis method to provide reference for etiology study and prevention of CHD .Methods The Chinese literature database such as CBM , VIP ,CNKI ,Wan Fang were retrieved from 2005 to 2015 for collecting the related literatures ,then the collected literatures were screened ,performed the information extraction ,quality evaluation and merged analysis by using the Stata12 .0 software .Results In this study ,37 articles were included ,including 20 provinces (municipalities and autonomous regions) ,8 588 cases in the case group and 12 479 cases in the control group;a total of 21 risk factors(5 pre‐pregnant factors and 16 gestational factors) were included . Conclusion All 21 risk factors include 4 pre‐pregnancy factors ,15 gestational factors and 2 factors which not be verified .The top 3 risk factors and their OR values with 95% CI were gestational diabetes 5 .80(2 .72-12 .37) ,contacting the occupational risk factors 5 .14(3 .30-8 .00) ,advanced age before pregnancy 4 .96(1 .45-16 .97) .Not considering to be as the risk factors and their OR value with 95% CI are high body mass index before pregnancy 1 .32(0 .99 -1 .75) ,living near street during pregnancy 1 .36(0 .50-3 .71 ) .

@#Objective To observe the effect of homemade ankle-foot orthopedic band on motor function of lower extremities in children with foot drop post spastic hemiplegic cerebral palsy (CP). Methods 24 spastic hemiplegic CP children were randomly divided into observation group (n=12) and control group (n=12). Both groups accepted roution rehabilitation training. The control group accepted walking training, and the observation group accepted walking training wearing the homemade ankle-foot orthopedic band, for 12 weeks. They were assessed with modified Ashworth Scale (MAS) of gastrocnemius, range of motion (ROM) of ankle active dorsiflexion, and D and E domains of Gross Motor Function Measure (GMFM-88) before and after treatment. Results The score of MAS significantly decreased in both groups after treatment (P<0.01), and decreased more in the observation group than in the control group (P<0.05). ROM and scores of D and E domains of GMFM-88 significantly increased in both groups after treatment (P<0.01), and increased more in the observation group than in the control group (P<0.05). Conclusion Walking training with the homemade ankle-foot orthopedic band may further decrease the muscle tone of gastrocnemius, increase the active range of motion of ankle and improve the motor function of lower extremities in children with foot drop post spastic hemiplegic CP.

Objective To explore the main risk factors related to birth defects to provide the scientific basis for making the best prevention policy and implementing the effective interventions .Methods Totally 21 domestic research articles on the risk fac‐tors of birth defects were comprehensively analyzed by the meta analysis method ,including accumulated 6 112 patients and 15 741 control cases .The articles were screened preliminarily according to inclusion and exclusion standard ,and then the fixed and random effects model were selected according to the homogeneity test .The merged results were performed the chi‐square test .Results The main merged results OR values of single‐factor‐analysis were as follows:the family history of birth defects 36 .22 ,early pregnant in‐fectious disease 5 .62 ,early pregnancy exposure to chemicals 4 .19 ,paternal smoking 3 .89 ,contact sedatives during pregnancy 3 .19 , pregnancy complication 2 .94 ,high protein food during pregnancy 0 .37 ;the main merged results OR values of multiple‐factor‐analy‐sis were as follows :early pregnancy infectious disease 7 .65 ,poor prenatal mental state 5 .44 ,early pregnancy fever 4 .70 ,early preg‐nancy exposure to toxic chemical 3 .90 ,history of abortion 3 .59 ,supplement of multivitamin during pregnancy 0 .45 .Conclusion The main risk factors of birth defects in our country are family history of birth defects ,early pregnancy infectious disease ,early pregnancy exposure to chemicals ,paternal smoking and pregnancy complication;the protective factors are eating more high protein food during pregnancy ,supplement of multivitamin during pregnancy and taking folic acid during pregnancy .

<p><b>OBJECTIVE</b>To investigate the effects of dexamethasone on the expression of interleukin-21 (IL-21) and phospho- STAT3 (p-STAT3) in a murine model of chronic asthma.</p><p><b>METHODS</b>Thirty-three female BALB/C mice were randomly divided into control group, asthmatic group and dexamethasone-treated group (n=11). In the latter two groups, asthmatic models were established by ovalbumin administration. The cells in the broncho-alveolar lavage fluid (BALF) were counted and classified. The airway inflammation was evaluated by HE staining, and the expressions of IL-21 and p-STAT3 in the lung tissue were detected by immunohistochemistry and Western blotting.</p><p><b>RESULTS</b>The total cell number and lymphocyte number in the BALF were significantly higher in asthmatic group than in the control group and dexamethasone group (P/0.01). Compared with the control group, the asthmatic group showed significantly increased protein expressions of IL-21 and p-STAT3 (P<0.05), which were reduced by dexamethasone intervention (P/0.05).</p><p><b>CONCLUSION</b>Dexamethasone can inhibit the expression of IL-21 and p-STAT3 in the murine model of chronic asthma, suggesting the importance of IL-21/STAT3 signaling pathway in the therapeutic mechanisms of dexamethasone for asthma.</p>

0.05). The data proved that PSMA7 was overexpressed in pcDNA3.1(-)/PSMA7-transfected A549 cell line, both in mRNA level and in protein level. Conclusion Eukaryotic expression plasmid pcDNA3.1(-)/PSMA7 has been successfully constructed, and the PSMA7 is stably expressed in A549 cell line. This would pave the way for further study of PSMA7.

AIM:To clarify if interferon-?(IFN-?), tumor necrosis factor-?(TNF-?)and interleukin-1?(IL-1?)can induce apoptosis of human airway smooth muscle cells (ASMCs) in vitro. METHODS: Human ASMCs were isolated and cultured in DMEM containing 10% fetal bovine serum. Passage 4-6 cell was used in the experiment. IFN-?,TNF-? and IL-1?, were used separately or together in the treatment of human ASMCs. The effects of IFN-?,TNF-? and IL-1? on the growth of the cells was detected by MTT method at the hour 0,24,48 and 72. Light microscopy and electron microscopy were used to examine the morphological change. DNA fragmentation was analyzed by agarose gel electrophoresis. SP immunohistological staing method was performed to detect the change of expressions of p53 , bcl-2 and bax gene. The apoptosis cell percentage were detected by in situ end labeling technique (TUNEL)of fragmental DNA. RESULTS: (1)IFN-? or IFN-? together with TNF-? and IL-1? decreased the number of viable cells in a time dependent manner. (2) Light and electron microscopic examination showed cell shrinkage, membrane blebbing, nuclear contraction, chromatin condensation and nuclear fragmentation in human ASMCs. (3) Agarose gel electrophoresis showed a characteristic"ladder"of DNA bands representing integer multiples of the internucleosomal fragments (about 180-200 bp) in cytokine cotreated human ASMCs. (4)The expression of p53 and bax gene in cytokine cotreated group was significantly higher than in control group, but the expression of bcl-2 gene was lower than in control group. (5)Stimultaneous treatment with IFN-?(4?10 5 U/L),TNF-?(4?10 5 U/L)and /or IL-1? (10?10 4 U/L) induced apoptosis of human ASMCs. AIM: To investigate whether leukotriene D 4(LTD 4) would stimulates proliferation of cultured human airway smooth muscle (ASMC). AIM To investigate weather and how aminophylline induces human trachea smooth muscle cells (TSMCs) to apoptosis. METHODS Human TSMCs were isolated and cultured in DMEM containing 10% fetal bovine serum. Passage 4~6 cell were used in experiment. The cells were cultured with aminophylline or 8 Br cAMP for 24 or 48 h. Light micros copy and electron microscopy were used to observe morphological change. DNA fragmentation was analyzed by agarose gels electrophoresis. SP Immunohistological staing method was performed to detect the changes of expressions of p53,bcl 2 and bax gene. The apoptosis cell percentage were detected by situ end labeling technique(TUNEL) of fragmental DNA. AIM: To investigate the influence and mechanism of recombinant macrophage migration inhibitory factor(rMIF) on fibroblasts.METHODS: MRC-5 fibroblasts were divided into two groups: the treated group was treated with rMIF(25-100 ?g/L,12 h,24 h or 48 h) and the control was non-rMIF treatment.The activity of proliferation in both groups was investigated and compared by CCK-8 means.Synthesis of collagen in the culture supernatants was detected by the hydroxyproline.The expression of collagen type I mRNA was examined using RT-PCR analysis.The level of collagen type I protein induced by rMIF was quantified by Western blotting.RESULTS: The production of proliferation ratio of fibroblasts treated with 50 ?g/L and 100 ?g/L rMIF at 24 h or 48 h were increased obviously(P
